I had some problem with party when I first started playing poker there and they wanted me to fill that application out to. I just called up instead and they verified the credit card I used to make deposits, my home address and called me at the phone number I had listed to make sure it was legit. They didn't do a credit check, or ask me for my full SS number or a checking account. Unless their procedures changed in the last 8 months I would call up instead of filling out that online form.
